Many new patients ask the same question. How does physiotherapy insurance Netherlands actually work in 2026? The rules look simple on paper, yet the details often surprise expats and long-term residents alike.

The Two Layers of Dutch Health Insurance
Dutch health insurance has two layers. First, the mandatory basisverzekering covers essential care for every resident. Second, the optional aanvullende verzekering adds extras like extensive physiotherapy.
Both layers matter for physiotherapy. However, only the supplementary plan reliably reimburses general physio sessions for adults. Therefore, knowing the difference saves money and avoids surprise bills.
Premiums for the basic package in 2026 range from roughly €145 to €165 per month. Supplementary plans add €10 to €45 on top, depending on the tier you select.
Basisverzekering: What It Covers for Adults
For adults aged 18 and over, the basic package excludes the first 20 physiotherapy sessions per condition. In other words, those sessions are not reimbursed from the basisverzekering.
From session 21 onwards, the basic package starts paying. However, this only applies when the condition appears on the official chronic list. Otherwise, you remain self-paying or rely on supplementary cover.
The deductible (eigen risico) also applies. In 2026 the standard deductible stands at €405. Consequently, the first reimbursed costs still count against that yearly amount.
The Chronic List: Cover From Session One
The chronic indications list, called chronische lijst, names conditions where physio is reimbursed from session 1. Examples include post-surgery rehabilitation, complex regional pain syndrome, and certain neurological disorders.
To qualify, your GP or specialist issues a referral with a clear diagnosis. Subsequently, the insurer may request a verwijsbeschikking or supporting medical letter. Without that paperwork, the chronic rule does not apply.
Even with chronic status, the first 20 sessions stay outside the basic package. Instead, supplementary insurance often covers those early sessions. Therefore, a good aanvullend plan remains valuable.
Children Under 18: Fully Covered
For children and teenagers under 18, the rules look very different. The basisverzekering covers up to 18 physiotherapy sessions per year per condition. Furthermore, no deductible applies to minors.
If more sessions are medically needed, an additional 18 can be approved. Chronic indications follow the adult chronic list but without the 20-session waiting period. As a result, families rarely pay out of pocket for child physio.

Manual Therapy Versus Physiotherapy
Many patients confuse manual therapy with regular physiotherapy. Both fall under physiotherapy in Dutch law, yet the practitioner holds an extra master’s degree for manual therapy. Consequently, the hourly rate is usually higher.
For insurance, both treatments draw from the same physiotherapy budget. Therefore, manual therapy sessions reduce the same supplementary session count as standard physio visits.

Aanvullende Verzekering: Tier Comparison
Supplementary plans typically come in three or four tiers. Basic tiers reimburse around 9 sessions per year. Mid-level tiers cover roughly 18 sessions, while premium tiers offer unlimited physiotherapy.
Reimbursement rates also vary. Some insurers pay 100% up to a maximum tariff. Others pay 75% to 80% of the invoice. Therefore, comparing fine print matters more than tier names.

How AlterPhysio’s Contract-Free Model Works
AlterPhysio is a contract-free clinic. In short, we do not hold direct contracts with Dutch insurers. Instead, we invoice you directly in English after each visit.
You then claim the invoice from your own insurer. Most Dutch insurers reimburse contract-free physiotherapy at 75% to 100% of their listed tariff. Consequently, your out-of-pocket cost depends on your specific policy.

Before booking, take three concrete steps. First, log in to your insurer’s portal and locate the polis voorwaarden document. Second, search for “fysiotherapie” and read both basic and supplementary sections.
Third, ask your insurer in writing how much they reimburse for a contract-free physiotherapist. Written replies protect you if the actual reimbursement differs later. Furthermore, screenshots of chat conversations also count as evidence.
If English support is limited, request the written reply in Dutch and translate it. Most large insurers, including Zilveren Kruis, CZ, VGZ, and Menzis, answer in writing within 5 working days.
Common Mistakes Expats Make
The most frequent mistake is assuming basic insurance covers all physio sessions. As shown above, the first 20 adult sessions usually fall outside the basisverzekering. Consequently, patients can face unexpected costs.
Another common error is switching insurers in January without checking supplementary cover. Premiums may drop, yet the new plan might cap physio at 6 sessions per year. Therefore, always compare both layers together.
Finally, some patients forget to submit invoices on time. Most insurers accept claims up to 12 months after the treatment date. However, earlier submission speeds up reimbursement and reduces administrative errors.
When to Pick Up the Phone
Insurance details change every January. Therefore, last year’s reimbursement amount is not a guarantee for 2026. Always confirm current rates before starting a new treatment series.
