Padel elbow is a tendinopathy of the common extensor origin at the lateral elbow. Essentially, it mirrors tennis elbow but with a slightly different stress pattern. Notably, the point of maximum tenderness often sits a few millimetres more posterior on the lateral epicondyle.

This shift reflects the kinematics of overhead padel strokes. Specifically, the bandeja and víbora shots load the wrist extensors in a flexed, ulnar-deviated position. Therefore, the extensor carpi radialis brevis tendon experiences shear that classic groundstrokes rarely produce.

Bandeja and Víbora: The Real Culprits

The bandeja is a defensive overhead with heavy slice. Meanwhile, the víbora is its aggressive sibling, struck with even more wrist snap. Both shots demand rapid forearm pronation under load, which spikes extensor tendon tension.

Furthermore, the glass walls force awkward off-balance returns. Consequently, players generate power from the forearm rather than the kinetic chain. As a result, the elbow absorbs forces it was never designed to handle.

How Padel Elbow Differs From Tennis Elbow

Classic lateral epicondylitis arises mostly from repetitive backhand strokes. In contrast, padel elbow stems from overhead and wrist-driven motions. Therefore, the dominant pain trigger is overhead loading rather than gripping.

Additionally, padel players often feel discomfort during the víbora follow-through. However, tennis players typically describe pain on backhand contact. Although both conditions involve the same tendon, the rehab plan must reflect these subtle mechanical differences.

Self-Assessment: Do You Have Padel Elbow?

First, press firmly on the bony bump on the outside of your elbow. Then, slide your finger slightly backward and downward. If sharp tenderness appears, your lateral epicondyle is likely the source.

Next, try a resisted wrist extension test. Specifically, straighten your elbow, make a fist, and lift your hand against gentle resistance. Subsequently, reproduction of pain at the lateral elbow strongly suggests padel elbow.

Finally, grip a coffee mug with your palm down. If lifting it triggers a sharp pinch, the diagnosis becomes even more likely. Nevertheless, professional assessment is essential to rule out cervical referral or radial nerve entrapment.

Treatment Progression That Actually Works

Importantly, you do not need to stop padel completely. Instead, we scale down volume and intensity while the tendon recovers. Therefore, load modification beats total rest in nearly every case.

Next, we introduce an eccentric loading program. Typically, slow heavy wrist extensions with a light dumbbell form the core. Moreover, isometric holds early on reduce pain within days for many players.

Grip size also drives elbow stress. Generally, a grip that is too small forces a death grip, which fires the extensors continuously. Meanwhile, an overly large grip blocks wrist mobility on the víbora.

Although padel uses pressurised balls rather than strings, string-like tension does not apply. However, paddle face stiffness behaves similarly. Consequently, softer EVA cores are gentler on a recovering elbow.

Return-to-Play: Criteria, Not Calendar

Many players ask how many weeks they need off. However, timeline-based return often triggers relapse. Instead, we use criteria-based milestones that reflect true tendon capacity.

First, you must achieve pain-free resisted wrist extension at full effort. Second, grip strength should match the uninjured side within 10 percent. Third, shadow swings must be symptom free for a full session.

Finally, we reintroduce padel through a graded match schedule. Initially, you play half-sessions with limited overheads. Subsequently, full bandeja and víbora loading returns once these markers are stable.
