Zorgtoeslag and Healthcare Costs: A Newcomer’s First-Year Checklist

Moving to the Netherlands brings a new healthcare system, and the costs can feel confusing at first. Furthermore, Dutch healthcare runs on mandatory private insurance, not a free public system. Therefore, every adult resident must arrange coverage within four months of registration.

This guide walks through your first-year checklist, including zorgtoeslag, the healthcare allowance. Moreover, we explain how physiotherapy and acupuncture fit into your coverage. As an Amsterdam clinic, we see expats navigate this every week.

What is Zorgtoeslag and Who Qualifies

Zorgtoeslag is a monthly healthcare allowance from the Dutch tax office. Specifically, it helps lower-income residents pay their basic insurance premium. The Belastingdienst transfers the money directly to your bank account.

To qualify in 2026, your annual income must stay below roughly EUR 38,000 as a single person. Additionally, couples have a combined ceiling near EUR 48,000. Your savings also matter, with a cap around EUR 141,000 for individuals.

You must be 18 or older, hold a valid residence permit, and have Dutch basic insurance. Consequently, students and expats on lower salaries often qualify in their first year. However, high earners and most knowledge migrants do not.

How to Apply for Zorgtoeslag Step by Step

First, you need a DigiD, the national digital identity. Therefore, request one as soon as your BSN arrives. The activation code comes by post within five working days.

Next, visit mijntoeslagen.nl and log in with DigiD. Then select zorgtoeslag and enter your insurer details, IBAN, and estimated income. The system calculates your monthly allowance instantly.

Submit before 1 September of the following year to claim retroactively. Otherwise, you forfeit the months you missed. Many newcomers lose hundreds of euros simply by waiting too long.

Basic Insurance: What It Covers

Basisverzekering covers GP visits, hospital care, most medications, and emergency treatment. In addition, it includes maternity care and mental health under strict referral rules. The premium typically falls between EUR 140 and EUR 165 per month.

However, physiotherapy is largely excluded from basic cover for adults. Specifically, only chronic conditions on a government list get reimbursed, and only from session 21 onward. Therefore, most musculoskeletal complaints fall outside basic insurance.

Acupuncture is never part of basic insurance in the Netherlands. Consequently, you need supplementary cover or self-pay. Read more on our pricing page for current rates.

The Eigen Risico: Your Annual Deductible

Every adult pays the first EUR 385 of basic-insurance care each year out of pocket. This is the eigen risico, or own risk. GP visits and maternity care are exempt.

You can voluntarily raise the deductible up to EUR 885 in exchange for a lower monthly premium. However, this only makes sense if you rarely use specialist or hospital care. Otherwise, a higher deductible costs more in the long run.

Plan ahead: physiotherapy and acupuncture do not touch the eigen risico, because they fall outside basic cover. Therefore, your deductible only burns when you hit hospitals, scans, or specialist prescriptions.

Aanvullende Verzekering: Supplementary Cover

Supplementary insurance reimburses physio, acupuncture, dental, and alternative care. Moreover, it ranges from EUR 10 to EUR 50 extra per month depending on the package. You can only switch insurer once a year, before 1 January.

Look for packages offering at least 9 to 18 physiotherapy sessions per year. Additionally, check whether acupuncture appears under “alternative therapies” with a clear euro limit. Common providers include Zilveren Kruis, CZ, VGZ, ONVZ, and Menzis.

Choosing an Insurer in Your First Month

Compare policies on Zorgwijzer or Independer before committing. Furthermore, look beyond the lowest premium and check the reimbursement tables for therapies you actually use. A cheap basic plan with no supplementary cover often costs more long term.

Pay attention to “natura” versus “restitutie” policies. With natura, you must use contracted providers, while restitutie reimburses any licensed therapist. Therefore, restitutie suits expats who want full provider freedom.

Newcomers on a 30 percent ruling salary often pick restitutie for flexibility. Meanwhile, students usually take cheaper natura plans with basic supplementary cover.

When Physiotherapy Saves You Money

Early physiotherapy often prevents costly imaging and specialist referrals. Specifically, a four-session course of root-cause physio costs less than one MRI co-payment under your eigen risico. Therefore, early intervention pays off financially as well as clinically.

Chronic back, neck, and shoulder issues respond well to structured physio plus targeted exercise. Moreover, combining physio with acupuncture shortens recovery for many overuse injuries. Read our integrated treatment page for details.

Acupuncture Reimbursement in 2026

Acupuncture sits under “alternatieve geneeswijzen” in most supplementary policies. Furthermore, reimbursement runs from EUR 30 to EUR 60 per session with annual caps. Always check whether your therapist is registered with NVA, ZHONG, or Zhong Yi.

Submit invoices through your insurer’s app or portal. Generally, decisions arrive within two weeks. Keep originals for at least five years in case of audits.

Tax Deductions for Healthcare in the Netherlands

Some healthcare costs are tax-deductible if they exceed a threshold based on your income. Specifically, this includes prescription medicines, certain therapies, and travel for treatment. The Belastingdienst calls this “specifieke zorgkosten.”

Keep every invoice, even small ones, throughout the year. Consequently, your annual tax return becomes easier and potentially cheaper. Box 1 deductions reduce your taxable income directly.

Common Mistakes Newcomers Make

Many expats forget to cancel their old foreign insurance, paying double for months. Additionally, some skip supplementary cover entirely and then need physio after sport. The result is unexpected bills at full private rates.

Another frequent error is failing to register with a GP, known as huisarts. However, the GP is your gateway to specialists and imaging. Without one, your insurer may refuse to reimburse hospital claims.

Finally, people underestimate the four-month deadline for insurance after registration. Therefore, fines and backdated premiums can quickly add up to over EUR 500.

Your First-Year Checklist

Within month one, register at your gemeente and request a BSN. Then apply for DigiD by post. After that, choose a basic insurer and add the right supplementary package.

By month two, file zorgtoeslag if eligible and register with a huisarts. Furthermore, save your insurance card and policy PDF in cloud storage. Keep them accessible from your phone.

Book early when pain or stiffness lasts more than a week. Moreover, do not wait for symptoms to worsen, because chronic issues take longer to resolve. Most acute problems clear in four to six sessions.
